I am responsible for informing my tattoo artist of any skin condition that may affect the tattoo process or procedure.

I do not have any skin conditions such as acne, scarring, keloids, eczema, psoriasis, freckles, moles, or sunburn in the area of skin where the tattooing procedure will occur.

I am not under the influence of drugs or alcohol.

I am not pregnant or nursing.

It is my responsibility to inform the tattooist of any rash or infection of the skin presently on my body.

Diabetics must inform the artist of this condition and be aware that the healing times are longer and may present complications.

I acknowledge that it is not reasonable for the owner, employee, or any authorized agent of Raven's Thorn LLC to determine the possible allergic reactions as a result of receiving a tattoo, and I agree to accept all risk and acknowledge that such risk is possible.

I acknowledge that infection is always a possibility of getting a tattoo.

I acknowledge that I have received aftercare instructions and i agree to follow them. I also acknowledge that not following my aftercare instructions properly may result in infection or healing complications.

I agree that any touch-up work to be done that is a result of not properly taking care of my tattoo will be done at my own expense.

I have been made aware that if my skin is darker in complexion that some colors will not appear as vibrant as opposed to having a lighter skin tone.

I acknowledge that a tattoo is a permanent change to my appearance. I acknowledge that alteration to the skin including but not limited to tattoo removal and plastic surgery may affect the appearance of my tattoo.

I affirm that I have no physical, mental, or medical impairment or disability that could affect my overall well-being as an indirect or direct decision to get a tattoo.

I acknowledge that I am eighteen years of age or older and that I have truthfully provided a valid government-issued photo ID to Raven's Thorn LLC employees or authorized agents. I consent to the application of the tattoo and to any actions or conduct of the employees or owner of Raven's Thorn LLC and accept the term of this consent form as it pertains to the tattoo procedure.

I consent to the use of any or all publishable content to include video, photo, audio used on any social or televised platform.

Tattoo Aftercare Instructions
It is required after you get a tattoo that you follow the instructions given to ensure your tattoo healing process will be successful. The following information is offered to you to provide a step-by-step process to follow in order for you to avoid infection or complications after receiving a tattoo. It is understood that, as a client, you may already have a method for aftercare that works for your skin type, known to you by previous experience with getting tattooed, and that's okay! Being consistent is very important, especially within the first few days of getting tattooed. As a reminder to you, keep the tattoo clean and moisturized! Follow the instructions below for the best results.
saniderm instructions
If Saniderm was used: The bandage you have been given to cover your tattoo is breathable, waterproof, and hypoallergenic. It is important to leave the bandage on for a period of 3 days minimum, but should not be left on for more than 7 days. During this time, you are likely to see a bit of plasma/ink under the bandage; this is absolutely normal, don't worry! Once the tenderness is minimal to nonexistent you may then remove the bandage. To do so, run lukewarm water over the area and work one of the corners with an unscented antibacterial hand soap. Then continue to standard aftercare info below:
AFTER BANDAGE REMOVAL
1. Wash your tattoo under lukewarm water using an antibacterial soap. Do not use any abrasive materials such as washcloths, loofahs, or soap that has any granules in it.
2. Rinse well.
3. Let air dry for 10-15 minutes in a clean environment (away from furry friends and sticky kiddos!)
4. Apply a thin, light coat of a breathable ointment or unscented lotion.
Repeat this process at a minimum of 3 times a day, but do not exceed more than 5 times a day.
SOME NOTES & COMMON Q'S
Avoid over exposure of UV or sunlight, as these will cause an increased potential of ink fading. Do not over moisturize; over saturation will cause complications. At the same time never let your tattoo completely dry out; this will cause scabbing which can lead to infection. Do not expose your tattoo to chlorinated, salt water, or any standing body of water, as this can cause infection and other complications within the healing process. This includes baths, pools, beaches, rivers, lakes, or anything else you might soak in. Running water only!
During the healing process, we advise wearing loose-fitting clothing in the tattooed area. No skinny jeans over your new leg tattoo, no watches over your new wrist tattoo.
QUESTIONS OR CONCERNS
It is always better to be safe than sorry! If you have any questions or concerns about your tattoo's healing, please reach out to the shop or your artist. If there is any severe pain, major swelling lasting more than 3-4 days, or sign of infection, we advise seeking medical treatment immediately as this could be signs of an infection.
