By submitting this form you are acknowledging, consenting, and agreeing to the following terms and conditions.

  • I am 18 years or older of age.

  • A deposit is required for all appointments. No deposit = no appointment. An email requesting deposit payment will be sent after submitting this form.

  • The deposit will hold your appointment time on your artist's calendar and will come off the total cost of the tattoo. 

  • If you need to reschedule your appointment, please notify us no less than 24 hours before your appointment time.

  • Deposits are non-refundable.
